These Two Problems Are Connected
Research consistently finds that erectile dysfunction is one of the strongest risk factors for premature ejaculation. The link is straightforward: a man worried about losing his erection tends to rush, and rushing becomes a habit the body learns. Anxiety drives both, and each one feeds the other.
Which is why treating only one often leaves men frustrated. If both are present, both should be addressed.
Erectile Dysfunction
Why the evaluation matters more than the prescription
ED is frequently the first symptom of a vascular problem rather than a standalone condition. The arteries supplying the penis are narrower than the coronary arteries, so they show reduced blood flow earlier. For many men, erectile difficulty appears years before a cardiac event.
That is why we take a history, check blood pressure, and run labs where appropriate before writing anything. A questionnaire and a pill in the post will not find that. A pill that works while an underlying condition goes unaddressed is not good care.
On-demand options
Sildenafil is taken roughly 30 to 60 minutes before activity, with a window of about 4 to 6 hours. It works best on an empty stomach, since a heavy or fatty meal delays absorption.
Tadalafil is also taken as needed but has a window of up to 36 hours, and food does not meaningfully affect it. The longer window is why many men prefer it — one dose covers a weekend rather than an evening.
Daily ED Program
A low dose of tadalafil taken every day rather than before activity. Nothing has to be planned or timed, and for many men that alone changes the experience. Sex stops being something scheduled around a pill.
Daily dosing also improves urinary symptoms from an enlarged prostate, so men dealing with both often do particularly well on this approach. It is the option most men are never told about.
Premature Ejaculation
What it actually is
Premature ejaculation means reaching orgasm sooner than you or your partner would like, with a sense of having little control over the timing, and distress about it. It is the most common sexual complaint among men, and it responds well to treatment.
How we treat it
Please read this carefully: no medication is FDA-approved in the United States specifically for premature ejaculation. The medications used are approved for other conditions and prescribed off-label for this purpose. Off-label prescribing is lawful, common, and supported by clinical evidence — but you deserve to know that going in.
Certain antidepressants in the SSRI class delay ejaculation as a side effect. In studies, that side effect has been used deliberately, with the majority of men reporting meaningfully increased time and improved satisfaction. They can be taken daily or, in some cases, a few hours before activity.
Topical anesthetic creams and sprays applied before sex are an alternative, particularly for men who would rather not take a daily medication.
SSRI safety information
Antidepressants carry a boxed warning for increased risk of suicidal thoughts and behavior in children, adolescents and young adults under 25. If you have any history of depression, bipolar disorder, or suicidal thoughts, tell us before starting.
Common side effects include nausea, drowsiness, dry mouth, and reduced libido — which is worth weighing, since it can work against the outcome you want. These medications should not be stopped abruptly. Combining them with other serotonin-affecting drugs can cause a serious reaction, so tell us everything you take, including supplements.
Combined ED & PE Program
Where both problems are present, we combine daily tadalafil with treatment for premature ejaculation. Many men do considerably better on the combination than on either alone, because the confidence from a reliable erection reduces the anxiety driving the rushing.
We prescribe these as two separate, standard medications rather than a combined product, which keeps your dosing of each one adjustable. If the ED dose is right but the other needs changing, we change only that.
In-Office or Telehealth
None of these medications is a controlled substance, so we can treat you in person at our Corona clinic or by telehealth in the states where we are licensed. Tell us where you live and we will confirm what we can do.
That said, there is a reason to consider coming in at least once. ED is frequently the first sign of a vascular problem, and blood pressure and lab work are worth doing properly at the start. An online questionnaire cannot check either. After that, ongoing management by telehealth works perfectly well — same provider, same plan.
Important Safety Information
Do not take any ED medication if you use nitrates in any form — nitroglycerin, isosorbide, or recreational “poppers.” The combination can cause a dangerous drop in blood pressure.
Tell us about every medication you take, particularly alpha blockers for blood pressure or prostate symptoms, which require careful timing. If you have had a recent heart attack or stroke, unstable angina, or severe heart failure, we will assess whether treatment is safe.
Common ED medication side effects include headache, flushing, nasal congestion and indigestion. Seek immediate care for an erection lasting more than four hours or any sudden change in vision or hearing.

Frequently Asked Questions
Which is better, sildenafil or tadalafil?
Neither in the abstract. Sildenafil is shorter-acting and more food-sensitive; tadalafil lasts far longer and can be taken daily. Most men find one suits them better and we adjust from there.
Is daily better than on-demand?
Daily suits men who want spontaneity, who have sex often enough for it to make sense, or who also have prostate symptoms. On-demand suits men who prefer taking something only when needed. Both work.
Will treating my PE affect my sex drive?
It can. Reduced libido is a known effect of these medications, which is exactly why we start low and check in rather than prescribing and disappearing. If it happens, we adjust.
Do I need my testosterone checked?
Often, yes. Low testosterone contributes to ED and to reduced desire, and treating the ED alone will not address that.
Can I get this by telehealth?
Yes, in states where we are licensed. These are not controlled substances, so the rules are more straightforward than for testosterone. An initial in-person visit is still worthwhile given how often ED signals something else.
Is any of this covered by insurance?
ED medication is often not covered, but generics have become inexpensive. We will be straightforward about costs before you commit.
What if the first thing we try doesn’t work?
