Three different problems on one neck
People say “my neck” and mean one of three things. They’re not the same, and they don’t respond to the same treatment.
Horizontal lines — the creases running side to side. This is what most people mean by tech neck, and it’s what we treat with injectables.
Vertical bands — the cords that stand out when you tense your neck. These are muscle, and neurotoxin relaxes them.
Crepey texture — thin, papery skin. This is a skin-quality problem, not a line problem, and injectables aren’t the answer for it.
Part of what you get at your consultation is being told which of these you actually have — because the wrong treatment on the wrong problem is money spent for nothing.
How we treat horizontal lines
These respond best to a combination, because the line has two components.
Neurotoxin relaxes the muscle pulling the crease inward, so it stops being reinforced every time you look down.
Filler lifts the crease itself, restoring the volume that’s been compressed out of it.
Tox alone softens the line. Filler alone fills a line that’s still being actively creased. Together they address both halves, which is why we typically use both.
How long it lasts
Neurotoxin: three to four months. Same as anywhere else on the face.
Filler: around a year.
They’re on different schedules, so most patients come in for tox a few times a year and top up filler far less often. We’ll map that out with you so you’re not guessing.
When injectables aren’t the answer
Sometimes we look at a neck and tell someone that tox and filler won’t get them where they want to be.
If there’s significant skin laxity — loose skin rather than lines — injectables won’t fix it, and we’d be taking your money for a result you won’t be happy with. What that neck needs is tightening. We offer ThermiTight and Morpheus8, both of which work on laxity in a way injectables can’t.
And occasionally the honest answer is that what you’re describing is a surgical problem. We’ll tell you that too. We’d rather send you to the right treatment than sell you the one we happen to be holding.
What to expect
A quick in-office appointment. Some redness and small bumps right after that settle within an hour, and a little bruising is possible.
Filler shows immediately and softens over the following two weeks. Tox takes about three days to start and two weeks to fully set in, so the complete result is around the two-week mark.
Who shouldn’t have it
Not for anyone pregnant or breastfeeding, anyone with a neuromuscular condition such as myasthenia gravis or ALS, anyone with an active infection at the injection site, or anyone with a known allergy to a botulinum or filler product.
The neck is a more delicate area to inject than the face, and it’s worth being treated by someone who does it regularly. Botulinum toxin products carry an FDA boxed warning about the toxin spreading beyond the injection site — rare with cosmetic dosing, but real, and we’ll go over it with you before we treat.
