A hormone therapy membership means someone is actually adjusting your dose. Most hormone therapy fails for one reason: nobody does.
You get a prescription, you get told to come back in six months, and in between you’re guessing. Estradiol that was right in March isn’t right in July. Progesterone that helped you sleep stops helping. Testosterone that fixed your energy starts causing acne. Every one of those is a dosing problem, and every one of them is fixable — if someone is actually paying attention.
That’s what a membership buys. Not the hormone. The titration, and the access to get it changed when it isn’t working.
Why not just use insurance?
You can. But there are things an insurance-based practice structurally cannot do:
- Adjust between visits. Insurance pays for visits, not for the message you send in week three saying the night sweats came back. Adjustment requires access that a billable-encounter model doesn’t fund.
- Treat you when your labs read “normal.” Perimenopause frequently doesn’t show up on a lab panel. Symptoms come first, sometimes by years. A practice that requires abnormal labs to act will tell you nothing is wrong.
- Prescribe testosterone for women. Most won’t. There is no FDA-approved female testosterone product in the US, so it’s prescribed off-label — and off-label prescribing takes a provider who is comfortable with it and does it regularly.
- Manage weight and hormones together. Weight gain, fatigue, and low mood are symptoms of both hormone shifts and metabolic problems. Treated by two separate practices, neither one can tell you which is which. We’ve done both under one roof since 2011.
The memberships
All three include your consultation, your lab interpretation, your prescriptions sent wherever you want them filled, and a provider who knows your history. They differ in how often you’re seen and how fast you can reach us — never in the quality of the care itself.
Memberships are billed every three months, which matches how your prescriptions are written. That way your care and your medication stay in step, and you’re never taking hormones without someone monitoring them. Cancel any time before your next renewal.

For someone whose hormones are stable and who mainly needs the prescription maintained and reviewed.

Getting your labs drawn — you pick
Hormone care runs on lab work. Where you live shouldn’t decide whether you can do this.
Walk in to a Quest location — no additional collection fee
We send your order to Quest Diagnostics and you go whenever it suits you. Quest has roughly 2,000 patient service centers nationwide, so this works everywhere we practice. This is the standard, and it’s what most patients do.
Have a phlebotomist come to you — available in many areas, fee applies
A licensed phlebotomist comes to your home or office and draws your labs there. Availability depends on your zip code, and the collection fee is charged by the mobile service directly — typically $35–$90 depending on your area. We’ll tell you whether it’s available where you are before you commit to anything.
Mail-in collection kit — where the other two don’t work
A kit arrives at your door and you collect the sample yourself with a small lancet. Convenient, and sometimes the only practical option. Fewer hormones can be measured reliably this way, and a small percentage of samples don’t yield enough for the lab to run — so we use this selectively rather than as a default.
One clinical note, because it matters
Fingerstick and venous blood don’t always produce comparable numbers, particularly for estradiol, free testosterone, and SHBG. If your baseline panel is drawn one way and your follow-up the other, a change on paper may be the collection method rather than your dose — and we’d end up chasing it. So we’ll ask you to stay consistent through a titration cycle. If you start with a venous draw, we’ll keep you there.
What’s not included
We’d rather you know this up front than find out later.
- Medications. Prescriptions are sent to your pharmacy and billed by them. Many patients use insurance for this, or a discount program. We’re not tied to any single pharmacy and never will be — that’s deliberate. This includes weight-loss medications: at the Optimization level your prescription is written as part of your plan, but GLP-1 medications such as Wegovy, Zepbound, and their alternatives are billed by the pharmacy and are not covered by your membership.
- Laboratory fees. If you have insurance, the lab bills your insurance directly. If you’re paying cash, your lab work is billed through us at our contracted rate — well below what the same panel costs walking in off the street. Either way you’ll know the amount before anything is run.
- Mobile phlebotomy collection fees, if you choose that option.
- Aesthetic and procedural services outside what’s listed in your tier.
Refills require an active membership. Prescriptions are written in 90-day increments, and we don’t refill outside of an active plan — hormone therapy needs monitoring, and that’s what your membership provides.
You’ll receive a Good Faith Estimate in writing before you begin, as required for self-pay patients.
Who this is a good fit for
- Women in perimenopause or menopause with symptoms, whether or not labs are “abnormal”
- Women whose provider declined to treat, or wouldn’t consider testosterone
- Men with low testosterone symptoms who want monitoring, not just a prescription
- Anyone who has tried hormone therapy and stopped because it wasn’t working — usually a dosing problem
- Patients managing weight and hormones at the same time
Who it isn’t for
- Anyone who wants a prescription with no follow-up. We don’t do that, and you’d be paying for access you’re not using.
- Anyone who needs a primary care provider. At the Optimization level we review your full panel and give you recommendations across common conditions, but we don’t manage acute illness, handle referrals and imaging, or take after-hours calls. Keep your PCP — we’ll give you things to bring them.
Common questions
What does the first visit cost?
$125 for the hormone consultation. That covers the visit, your history, and getting the right labs ordered. If you enroll in a membership, the $125 comes off your first payment — so it only costs you anything if hormone therapy turns out not to be the right fit.
What will my labs cost?
If you have insurance, the lab bills your insurance directly, the same as any other lab order. If you’re paying cash, we bill you at our contracted rate — considerably less than the retail price of the same panel. We’ll tell you the exact amount before anything is drawn.
Can I submit any of this to my insurance?
The membership fee itself generally isn’t reimbursable — that’s true of any membership-based practice. We can provide a superbill for your visits, which some plans will apply toward out-of-network benefits. Whether yours does depends on your plan, so check your out-of-network coverage before you count on it.
Why is it billed every three months?
Because that’s how your prescriptions are written. Ninety days of medication, ninety days of care. It keeps the two in step so you’re never taking hormones without monitoring, and it means fewer payments to keep track of.
Can I cancel?
Yes, any time before your next renewal, and you keep your membership through the period you’ve already paid for. Refills stop when the membership does.
What if I start and it isn’t right for me?
Tell us. Adjusting the plan is the point of the membership. If the membership itself isn’t a fit, contact us and we’ll sort it out.
Can I use my HSA or FSA?
Ask your plan administrator. Rules for medical memberships changed recently and vary by plan, so we can’t tell you yes or no — your administrator can.
Do I have to buy hormones from you?
No, and we’d rather you didn’t feel obligated. Your prescription goes wherever you want it filled.
